in my country our packet 3 is just a request to send in the DS-230 part 1 so they can issue me an interview date and then i use that appointment letter to do medical and other stuff. You just need to find out what exactly is included in your packet 3 and respond to the consulate. sometimes these things get messed up in the mail. Ask within your regional forum

Yes, I have seen others mention getting misinformation when calling them. Just go ahead and follow the instructions, fill out the DS-260, pay the fee, schedule the appointments and get the other forms filled out and ready to go. Show up early at the consulate after you arrive in Juarez and ask to have the letter printed out for you because you never received one and need it for your ASC, medical and interview.
