i have question about the immunization records of the kids,how important is it on the medical exam? i lost their immunization card when we got flooded before and i go to the health center where they got their shots when they are babies but they cant provide me another card,just a piece of paper stating the dates of their shots.is this gonna be a problem for the kids medical examination?and they dont get a shots for hepa also is this gonna be another problem?pls advice?

My kids are just about to begin the process of getting the immunisations they need for the immigration process (they need ones that they do not offer as standard in the UK) and, when the process for them and me is complete, I will be getting my Doctor's surgery to print off a document for each of the five of us that states what immunisations we have had on what dates. As far as I am aware, that will be sufficient for the purposes of the medical. Of course, I am in a different country but I am sure the principle remains the same.

Married a US/UK dual national in 1996 and had four children together.
Immigration Timeline: I130 Approval November 2012; Interview July 2013; Immigration October 2013. (Note, however, that we chose to stall the process for personal scheduling reasons)
As a family of six, we relocated from Argyll in Scotland to Pennsylvania in October 2013. 

I applied for Citizenship in October 2017 and am currently waiting for an Interview date.

This is what the SLEC website states:

SELF-REPORTED VACCINES WILL NOT BE HONORED.

Your vaccination documentation will only be honored if it contains the following details:

personal copy (e.g. baby book)or copy of medical chart indicating received vaccines

complete date of receipt of vaccination

must include signature (license number, if applicable) of doctor or health worker who administered vaccine

A printed report produced by a medical physician would surely be replicating the information that would have been on the original immunisation record in any case so I assume that a print out, certified perhaps by a doctor's signature, should suffice in place of the lost card.

In my case, my children have all the shots they are required to have for the UK and, therefore, their record cards are filled accordingly. There is no place on those cards for all of the additional vaccines they are required to have in order to emigrate to the US since they are not required to have them in the UK. That is why I will be taking the printed records to the medical.

For my own part, my vaccination record is complete and up to date but I possess no immunisation card because my parents lost it decades ago. All of the information on my past immunisations and experience of diseases (such as chicken pox) forms part of my medical records and I am going next week to get the shots I need for emigration that I would not be required to have normally. I too, therefore, will be taking a print out that shows all of my vaccination history from the 1970s (ie when I was born) onwards.

Additionally, the chicken pox and shingles vaccines are not available in the UK so I cannot possibly fulfil that requirement of the medical. However, it should be recorded in my medical notes that I have had chicken pox (as have my four children) and even if it is not I will basically self-certify that the kids and I have all had chicken pox. Perhaps they can draw blood to check for our immunity if such a thing is possible for that disease (as it is for others). Also, it may be the case that my kids have not had their third and final round of the Hep B shots prior to the medical but I will get a doctor to provide a letter stating that they are scheduled to receive them by X date and hopefully will be given a waiver on that basis since it is not medically possible to speed the scheduling of those shots up for children any more than is already being done.

I hope all of that made sense.

Basically I think you should be perfectly OK with a printed record of your children's immunisation history that has been certified by a medical clinician.
