Hi again VJer's!

I'm compiling all our papers for our AOS.. Just a question, how do we obtain a Form I-693?? I already had my vaccination back in the Philippines before my Interview at the Embassy. Do I still need to get this Form from a Doctor? :unsure:

Can't wait to spend the rest of my life with you..

05/30/2012 - Interview - Visa Approved!

Timeline

If your medical was done within the last year and all of your vaccinations were complete, then no need to redo medical.

Read step #11 on this guide

Thanks! So it means I don't need the Form I-693 anymore? Do I still need to provide them with my Completed Vaccination Form (for the AOS) ? :blush:

I read some post here that they didn't fill up that I-693,they just send the vaccination documentation worksheet from St lukes,it was too late for me to know that b'coz we already went to see the civil surgeon and he transfer my vaccination record in my I-693 form and pay him for $45.i had my medical in St lukes last april and filed my AOS last july so it was only few months,my medical still valid..some civil surgeon here charge you more...my friend in Florida pay $260,and give her another shots she said it's better to do it than to get RFE for not including the I-693...

July 30'12 received mail NOA1(AOS/EAD/AP)

Aug.22'12 Biometrics done(Detroit,MI)

Sept.19 USCIS mailed a noticed they approved my Travel Docs.

Sept.20 EAD card on production(approved in 60 days!)

Sept.25 USPS pick up mail from USCIS(EAD card on the way!)

Sept.27 EAD card received.

Sept 28 went to SSOffice for name change on SSN card.

Oct 05'12 Got my new SSN card.

April 16'13 USCIS emailed Case Status:Card/Document Production.

April 20'13 Recieved mail from USCIS welcome notice.

April 22'13 USCIS email that USPS picked up mail containing my GC.

April 25'13 Green Card on hand,finally!...

Ouch that's quite a lot if there's an Additional Charge.. and CS sometimes charge more than what we expect. I guess i'll just provide my Vaccination Documentation on my AOS I've read some and it didn't cause them any RFE..

Did you fulfill the vaccination requirements when you went to the panel physician for the K1 medical?

You should have a copy of all of your vaccinations with you. If you have fulfilled the requirement and the panel physician checked them off, then nothing else needs to be done. It is in the medical record.

If, you have not fulfilled the vaccination requirement, you will need to go to a civil surgeon and have them complete the form for you and send in the sealed envelope. The problem is most civil surgeons want to charge the full price of a medical exam, not pro-rated just for vaccination part.

I got no RFE, and received NOA for an interview today :)

Did you fulfill the vaccination requirements when you went to the panel physician for the K1 medical?
