We are just starting our AOS, and we had a question about the I-693A. How do we go about getting this?

You make an appointment with a "panel" (can be found from USCIS list) civil surgeon, and show the consular (for K-1 or K-3) nonimmigrant visa if it was within the prior 12 months; the physician will determine if shots (or titres) are needed and mark these down. You will get two copies, one in sealed envelope (to be given to USCIS at interview).

Yes, you don't need to print the form I-693A, the Civil Surgeon office have that forms.

I suggest you call around to as many civil surgeons as possible in your area, the fees can vary by a considerable amount. I would also suggest you take the xray from the embassy physcial with you too. The CS we went to wanted to do another xray so I whipped out the one Claudeth had from the embassy and saved us thirty five bucks. Good luck
