parental advice for cfo seminar

i have read in this forum that i need a parental advice to marry. my question is where am i going to obtain this form and does it need to be notarized? my parent is outside the country fyi....

I don't really know where to get the form. Have you tried city hall, or the civil registrar? Hopefully, someone with experience will chime in.

Quezon City's form for those who are 18-21 years old looks like this.

The form for those who are 21-24 years old looks like this.

If one of your parents can't sign the form, you can get a legal guardian to sign it. The form also seems to allow a "person in charge" to sign it. :)

kristinebelle i have read that you already had your parental advice.. how did you exactly do it? do you get it from cityhall or you just printed a sample form of parental advice and have your parents signed it and notarized?

please post something about your experience regarding parental advice on your seminar so that i have an idea. it would help a lot! thank you.

elmer and hensie i just went to a lawyer and let him do it and signed my mom i put my fiance's name there his address and all...

i will be having my seminar at PRISM this monday..when are you going to attend yours?

For the purposes of a K-1 applicant going through CFO, you will probably be fine with your lawyer doing this. I mentioned in your other thread that I would use the forms over hearsay of writing it out yourself. However, it is unclear if that form from Quezon is city specific although there is a part on the form asking for "City or Municipality". Davao doesn't seem to have a link to those forms. They mention either going to LCR to complete the form or have a parent write a notarized statement. It seems either one format would be acceptable if notarized, maybe? On that note, this is usually meant for people marrying in the Philippines so LCR being involved seems irrelevant anyway. The CFO is likely the only time a K-1 applicant would even need to consider this. Since you already have a lawyer, he/she should know better. If CFO gives you a hard time or tells you different, take it up with your lawyer.

I think you are fine. You stated earlier you had it notarized by a lawyer. If your parent/s participated, all signatures are there, and it is notarized, I don't see why CFO would give you any problems. Just follow the lawyer on this as this is Philippines law, and he/she can give you that confidence.

CFO can be such peckers! Too much power given to people with little control...

Required documents & "OTHER DOCUMENTS" they ask for at CFO: copies of; CENOMAR, certified birth certificate, bio page of passport, visa page from passport, pictures together, from USC - bio page of passport, birth certificate, divorce decree (if been married before) are the most common. The "other documents" clause can be ANYTHING with those nit-wits at CFO.
