I have read the pinned thread and am still unclear. Here's my situation. To obtain my fiance's K-1 we got her initial physical, vaccinations and chest X-Ray in Bangkok at Bumrungrad August 2011. This was handed to us, sealed and this was turned in to the Bangkok U.S. Embassy. While all that process was going on we got (2) more vaccinations in Thailand (not Bumrungrad) as outlined from our Bumrungrad doc.

Fast forward and now we are in the U.S. and compiling my AOS docs to turn in. Let's assume my original medical docs were not lost by USCIS and that I'll be able to turn in my AOS stuff before a year from her last medical. Assuming that; USCIS will be looking for 3 vaccinations which (2) I have already done and (1) that I will do in the next couple of days.

My question: What do I need to get a U.S. Civil Surgeon to do for me ? I-693 ? Will the U.S. Civil Surgeon accept my vaccination docs from Thailand ? Will they try and get me to do a new physical ($300+) ?

The docs were not lost by USCIS. They will be forwarded to the office where you will interview. If the timing is right you wont need a C S at all.

In my case they didnt ask nor care about any additional shots. I suggest you do as I did and use your local county health clinic for the follow up shots. I had 3 wich cost 23 bucks. My daughter had a total of 9 which were free.

็Hey Ning

If you read again my post it says "assume they were NOT lost" So are you saying do not even do a I-693 ? Also I think the packet with the medical stuff is first sent somewhere other than the place where the interview will take place. My wife needs only (1) shot left and she will have it here.

If you had all the vaccinations that are required for AOS at the time of your medical, then you do not need to see a CS for the I-693. If you did not have them all then, and got some after the medical and DS-3025 was filled out there, then you need to find a CS and get them all transcribed onto an I-693. She would have needed to get at least one MMR, one Tdap/TD, varicella or history of having chickenpox, and a flu shot, if it was flu season at the time. You only need one in a series, and not the whole series to be complete for filing AOS. If she had those at the time of her medical, then you can skip the CS. If she got any of those after the medical, then she needs to see a CS. You will most likely need to call around until you can find one that won't insist you need a new medical. You can print out the instructions that state you do not need the medical if you entered the US with a K-1.

Very informative. Thanks.

So in my case we had (2) shots after the "medical and DS-3025" and will have (1) more shot for a total of (3). So basically all I need is for a U.S. CS to certify and transcribe these (3) shots onto a I-693 ?

Anyone know where I can download a I-693 ???

usually any civil surgeon that is certified by the USCIS will carry the copy of I693 at their office

I did my physical 2 weeks ago, all I brought with me was just my ID and immunization record. They have everything there.

If you still want to download, here is the link : http://www.uscis.gov/files/form/i-693.pdf
