are the interviewer or CO do an investigation in each case they are handling? before they approved nor deny it....

my case on my interview last april 18 the CO said visa approved but i dont have my visa until now cos my status is under admin review which means security name and background check.

I would say all cases get investigation, some get AR.

More precisely, the CONSULATE does an investigation of each case. The interviwer does not, they interview. There will be some investigation done before your interview and possibly some after (this is called AP or AR) The consulate will use whatever and whomever they deem necessary to check your case.
