I am trying to get the sworn affidavit from ex-wife to submit my RFE. need help what should be included in the affidavit.

It is very good idea & strong evidence. Basically she should be saying "the marriage has been entered in a good faith & the didn't work out".

She can write any format she wants, it doesn't have to be notarized.

"Patience is the key to the paradise (US Citizenship, in our case)"
