Do you need to have a I-693A to send off with the other forms or will a copy of the vaccination supplement done at the medical suffice? The reason i ask is beacause everytime i call the civil surgeon listed for this area off the uscus website, it always goes to voicemail...i have left messages but a week later still nobody has got back to me...i have been there and they wont even let you ask a question without an appointment which you need to get by telephone.

I did a google search for civil surgeons in my area then called a bunch and made sure they were approved by USCIS and asked how much they would charge to have this done. I ended up having to drive an hour but it only cost 10 dollars. Try calling around even to any doctors office because usually the doctors know eachother in their area and can probably point you in the right direction. I am not sure what would happen if you sent just the record you have now- I guess either they will accept it or you will get an RFE.

Timeline

The decision as to whether or not to get the Vaccination Supplement done is totally up to you, but here's what I have experienced (and which I have also noticed from other VJ members posts).

When I applied for my own AOS, I sent along a copy of the "Vaccination Worksheet" from my K-1 medical, and was never asked for anything else and got approved no problems.

When I applied for my daughter's AOS (much later, because she followed-to-join), I also sent along a copy of her "Vaccination Worksheet" from her K-2 medical. I received an RFE asking for the I-693, etc., so I found a civil surgeon who copied the information from her worksheet onto the Supplement and took it with us to her AOS interview. At her AOS interview, the Vaccination Supplement was NEVER asked for. I had to offer it to them at the end of the interview before we left because I just figured they may as well have it for her file since I had paid to have it done. But, my point is, if I hadn't had the Supplement done, it would not have been a problem.

I have also noticed other VJ members stating that their AOS was approved, using only the Vaccination Worksheet and not the Supplement.

i did the same thing... included my vacc worksheet (which originated from medical) in my AOS package . i was approved and supplement was not even brought up during the interview (though i was ready with it).
