Hi Folks,

I am filling the I-495 (K1 fiancee category) - Got a couple of questions (might sound retarded) but hey better be sure than sorry :)

Part 1

1. Last Name = Married Name (ie no more Maiden Name) I took my husband's last name and it appears on our marriage certificate

2. Any of you got an A# ??

Part 2

1. Section C "Attach a copy of fiancee petition approval notice and marriage certificate" - which one is the petition approval? Would the K1 visa on the passport enough ??

Part 3

1. Non Immigrant Visa Number - am assuming it's the number on the K1 visa.

Part 5

1. Signature - Again it should be Married Name !??

Yes, or course you use your married name.

Yes, you have an A number. Its also on your Visa (along with your Visa number).

The petition approval is the notice (I-797C) from the USCIS saying your petition was approved. No, you don't attach a copy of your visa.

Yes Married Name

Written on the NOA-2 (I-797)

Part 2

1. Section C "Attach a copy of fiancee petition approval notice and marriage certificate" - which one is the petition approval? Would the K1 visa on the passport enough ??

NOA2 - is the petition approval (you need a copy)

Part 3

1. Non Immigrant Visa Number - am assuming it's the number on the K1 visa.

Correct

Part 5

1. Signature - Again it should be Married Name !??
