There is a question "Have you obtained a certificate of citizenship in your name?" Does this mean if I have received a certificate with my name in behalf of someone else. The first one applies to me because I got mine through Naturalization. I'm confused on that one it says YES or NO. Or do I put YES and put the same my certificate number and location where issued???

For #10 you would check the Naturalization box and enter the certificate #, place and date of issue in the box below. Leave the rest for #10 blank. That question is pertaining to gaining citizenship through your parents, which does not apply to you.
