What filling the documents for AOS what name should be used for the Forms?

Should her Maiden name be used or her new married name?

Or do certain forms need the new name and the others should be Maiden name?

As is always said on this question - You use the name that she is going to use. If she is using her married name from now on then fill out everything in married name to get the green card in married name.

Use her married name except in the places where the forms ask for "former name" or "name as it appears on I-94"
