I am confused on what info to put on line 25 Federal income tax information. On Line 24, my own current income is above 125% of poverty line, so I do not need /do not intend to include spouse's income. However, 2009 we filed joint return, instruction said for the purpose of this form, should include income from gross total income. If I use the gross total income from the joint return, then it will include spouse's income. So what do I do? enter the income straight from 1040, or only enter the w-2 income for myself for 2009? Anyone has experience on this one? Tks.

just put your income and send copy of your 2009's W2 they will know what you alone earned. and you haven't filed jointly every year?

Thanks for your reply. No, I didn't file jointly every year, since I will only be sending in the most recent year tax return as required, which is 2010, I will not send 2009 return, therefore, i will not send in w-2 for 2009, so what should I do, the form clearly says enter the gross income from 1040.

Enter YOUR gross income from the 1040. You would have entered the gross income from your W-2 onto the 1040 at some point as well as your wife's gross income and then combined them. Don't include the combined amount, include your number only. If you aren't sure which line you entered it on, check the 1040 instructions.
