I have (2) Questions about the I-864EZ

#1) Country of Domicile: I am a (USC) and have always lived in America since birth, What do I write in box 10 for country? or do i leave it blank?

#2) Question number #19 say's (adjusted gross income on IRS Form 1040EZ),Is the I-864EZ form only for people that filed Form 1040EZ with the IRS? I ask because 2011 i filed 1040.

You would write United States of America or USA. Do not leave it blank.

No, you can use the I-864EZ form if you filed taxes using the 1040 form.

Gotta read the whole sentence. It tells you "total income" from a 1040 or adjusted gross IF IF IF you filed the 1040EZ, BECAUSE there's no "Total Income" line "22" on a 1040EZ.
