On number 7 where it asks about income, deposits, property and other personal value...

Does other personal values such as vehicles need to have a copy of ownership and a appraisal such as kelly blue book value?

For life insurance does my co sponsor need to summit paperwork stating how much?

If the income is sufficient on its own, meaning the person makes 125% of the poverty level or greater, then there is no need to fill out the assets section at all. Just leave that stuff blank.

If you need to use assets, then yes, everything must have documents to prove existence, ownership and value. Also, if using assets to make up a shortage in income, it needs to be 3 times the amount that you are short. Example: You need $20k, but you only make $18k, you need 3x the $2k you are short, or $6k in assets.
