Never Given DS-3025?

When and where should we have gotten the DS-3025 form? Is it possible that immigration took that form when she entered the US? She said she gave them papers when they took her into a room to process her visa.

The only record of the medical exam that my fiancee has is a vaccines record that the clinic gave her but that is in Spanish. I was hoping to just send in the DS-3025 instead of going to a Civil Surgeon but that doesn't seem to be a possibility if we dont have the form. Will the Civil Surgeon accept this vaccine record that we have if its in Spanish? All the medical terms are universal.

Would appreciate any help.

I did not get the DS 3025 either and I had too my vaccination record (not the one from the clinic, just the 'official' record that people usually get in Mx). It was in spanish and I took it to the CS and that worked just fine, they did the transcripts for me. find a CS that is familiarized with Immigration and with the K1 process so they won't try to charge u an arm and a leg to fill out a simple form.

You have to ask for it from the medical facility where you did the medical.
