MAKE A CALL first before you go'google it"and get thier phone number because there are specific day for country to country;

my experience is I brought NBI.Baptismal cert,b-cert.approval letter from embassy with case number,passport,2x2 picture (for sure)make a photocy of everything u are bringing (2 each)there is payment also check it out now they have office infront of NCBA its more easier there from what i know...good luck.

I took my CFO in Cebu and they ask for 2 valid ID's and a copy, Birth certificate NSO paper and a copy, picture together,

Original and a copy of passport, Legal Capacity (if ur married) a copy of your husband/fiancee passport with arrival stamp.

And Php 250.00 for the seminae fee. Goodluck!

A couple additional tips, my fiance said when she was there many had to call their boyfriend to answer some of the questions on the forms like his moms maiden name, complete address in US etc, better to just bring that with you. You can go online to the CFO website for schedule and required documents and things. You can take the seminar before you get your visa but you will still have to go back and get the sticker in your passport after you have visa. Jim. http://www.cfo.gov.ph/
