Once you receive your permanent resident card and change addresses. The form ask "Port of Entry Into US" & "Date of Entry Into US". Do you use the last port your entered or do you use the office location where you interviewed for the PR card?

Thanks.

The port of entry is typically the first airport where you were entered the USA where your I-94 card and passport was stamped.

Date entry will also be in your passport.
