If you filed for AOS in your married name then it should be in your married name.... if not then in your maiden name.....

I hope they are not too strict about this. In our case, we had the vaccination supplement filled out before we got married so the form has my wife's maiden name on it, but we submitted AOS in her married name. I did include her maiden-name on the AOS forms and G-325 where asked, so hopefully it won't cause a problem. I guess I can always go back to the doctor and get them to fill out another form in her married name at the time of our interview.

We had Civil Surgeon fill in doc in Full NAME (First / Maiden / Married) so there would be no question!
