Most cases are denied at interview, few petitions are denied unless there is a very clear problem (such as bigamy).

The % denied is difficult to say- it depends on your beneficiary's country as well as what you consider to be a "normal" case. In most situation, you get a chance to "make things right" and submit more evidence before the case is actually denied.

I am unclear what your question is.

Are you asking whether most visa applications are denied? Definitely not.

Are you asking what the most common reason for denial is? Not having/ not being able to prove a bonafide relationship, ie the Consular Officer suspecting you only want the visa to get a greencard, not because you are in a genuine relationship.

Are you worried about anything specific?
