I have searched the forums for answers to these questions but I couldn't get the correct answer for a few specific ones. And I want to ask a couple of the same questions just to re-confirm.

Btw, I'm an An American-born citizen. My wife is from Indonesia.

1. Her family does not have a "family name." So in the FAMILY NAME boxes, what should she put?

2. What should both of us put in the FILE NUMBER boxes? Some have said to leave it blank or put "None".

3. At the very bottom of the heavy line box, do we need to fill that out if we both do not have an alien registration number? or is that just for people with alien registration numbers?

I'm just a little nervous that a form may be filled out incorrectly. And that is why I am asking for myself these popular questions.

4. in the "Last occupation abroad if not shown above" should my wife and I put "None" or "N/A"?

I've been reading some of the searched threads and there is confusion which to use. Someone said that an application could have a flag raised if "N/A" was used. :huh:

Suggestions: Do not leave anything blank. Either N/A or NONE (everything capital letters).

My suggestions are based on my answers and mine was approved with no problem.

Yes, never leave anything blank - answer NONE or N/A, as the case may be.

08/30/13 - Biometrics Done - No walk ins allowed at this LSC (received on 8/16/13).

12/28/11 - Received SSN (applied on 12/20/11, as we didn't get based on DS-230 options)
11/28/11 - Received Green Card (Expires on 10/30/13) - Welcome Letter on 11/17/11
10/30/11 - POE - Houston, TX

Chennai Consulate (40 days)
10/28/11 - Received Visa papers and Passport at VFS
10/25/11 - Interview Cleared Successfully (Spouse was not allowed in)

NVC: (90 days from NOA2 to Consulate)
08/31/11 - Case Completed (Interview 10/25/11) - Received at Chennai on 09/19/11
07/22/11 - NVC Case Number

USCIS: (92 days)

6/21/11 - NOA2 (NOA1 on 3/25/11) - took a month to get to NVC
3/21/11 - I-130 sent to USCIS Lockbox, Chicago.

She did not add her last name after we married. Thank you for your answer replies :) I will ensure that I don't leave anything blank.

And just to let you all know, I've also emailed the US Embassy in Jakarta and they have told me to use her middle name as her family name. My wife's friends have said to leave it blank. Hence why I need to get a 3rd or rather as many opinions as possible.
