I-94... never received one?

My fiance never received an I-94 upon crossing the border to come stay with me as a visitor while we file paperwork. He crossed via car and then took a flight from Buffalo so they never gave him the declaration card. Will they return my visa if I don't have the number?

No, and you did not file for a visa yet. You filed a petition that establishes a fiance relationship. Relax :)

Yeah the I-94 number drove us crazy too when we came to that part of the paperwork but we found a link that said Homeland Security were moving to becoming less reliant on paperwork, printed it off and attached it to the paperwork for the avoidance of doubt;

http://www.dhs.gov/ynews/releases/pr_1274366942074.shtm

"Release Date: May 20, 2010

For Immediate Release

Office of the Press Secretary

Contact: 202-282-8010

Washington, D.C.—Department of Homeland Security Secretary Janet Napolitano today announced the elimination of the paper arrival/departure form (Form I-94W) for authorized travelers from nations participating in the Visa Waiver Program (VWP)—streamlining secure travel for millions of visitors to the United States every year by consolidating the collection of traveler information and enhancing security by automatically providing DHS with important passenger information prior to departure."

We sent our paperwork in with copies of my ESTA for both recent trips.

Canada is actually not a part of the VWP and does not do ESTA. They enter under the terms of a B2 visa without having to apply for one. They are often not given I-94's, but they would never be given a I-94W.
