A post from someone (?) though not in Germany said they were able to have their medical without an NVC number but the doctor would not release the pack/results to them until they were able to give the number. In other words they were able to pay and have their appointment and tests after they were approved but before receiving their NVC number. You could check if that's possible with the doctor since there is currently a delay between NOA2's out of CSC and receipt of NVC number. It may not be exactly what you're asking, but you might as well wait until your NOA2 before spending the money on the medical and still save time in the overall process.
