Hey Folks. I did a search for my issue but it was mostly older posts and I would rather get the latest information.

Here is my question(s): Does I-134 have to be notarized? The instructions on the USCIS website say it doesn't but old posts say that it does. Please clarify!

Also, there is this field where I don't know what needs to be entered. Older posts say to leave it blank but I just want to be sure. Please see attached!

Example shows question 11 as neither Intend nor Do Not Intend. It says to put N/A (K1 Visa Process for Permanent Residence)

As far as I can tell it seems to be Embassy specific! USCIS is clear in that is not required; however the Embassy site for Warsaw says that it must be notarized. SO, since they the ones actually giving the visa, I'm not taking any chances and sent my fiancee 2, a notarized one and one is not...cost nothing except a visit to your local bank.
