Question about Form DS-156

The DS-156 is the form that must be filled out and sent to the embassy electronically. On this form, on the right hand side of the page, is a note that reads, "Please staple or glue photo here". Now, of course, we can't staple or glue anything to an electronic form ( :P ) but I was wondering, do we have to submit a photo electronically along with the form? I don't see anything in the instructions on the packet 3 that indicates that a photo must be submitted, however, my fiance had to submit a photo electronically when submitting a similar form at the time he applied for his B2 visa. What do you guys think?

Hi, thanks for the reply. The DS 156 is submitted to the embassy online before the embassy even issues the interview date. This is mandatory for all K1 visas as far as I know. I just don't know if we have to attach a photo electronically along with the form.

I guess they have different procedures.. We take our DS forms with us to our interview, other countries send the forms back to their embassies.. But as far as i know, the only form sent electronically is the DS 160.. K1- Visa applicants dont need that.. but, thats what i know.. maybe other VJers can answer that :)

My concerned is that the DS-156 form could be accepted if it filled out out by hand written? I have a problem filling it out, it won't allow me to continue when i answered in question no: 14(National identification Number) was not applicable and question No:15 in my home address has no apartment or house number coz i lived in a sitio only not in subdivision. The system would just said INCORRECT and it won't allow me to continue. Are the Street address line 1 and Street address line 2 can be the same Address? And the DS-156 was expired on May 31,2011,does that date still accepted? or is there DS-156 form, updated?..please write me back. Thanks and God bless!

Im almost sure in Brazil we need to attach a photo electronically along with the form, and then print it out.

But I'll e-mail the embassy a few days b4 the interview to confirm.

If I were you I would also e-mail them to check, or maybe u can do both? Print 2 forms ... one with the photo attached electronically and another one with the photo stapled or 'glued' (dunno if this word exists lol). So when u get there, they will tell you what they need indeed! :)

Good luck!

Dave & Carol :)

Well. the DS-156 form is filled out electronically and the bar code is generated. However no information is sent to the embassy. You just need to generate the barcode - that's why you fill it out on your computer. You add the photo and bring the form to your interview. Then they take it from you and scan the barcode - and all the info is transfered to their computer.
