First of all I want to thank all those who reply -- You have been very helpful with being patient and helpful while my fiancé and I figure out how to do these forms properly... so many questions lay unclear, common sense open to interpritation.

Question 1. "Family name in CAPS"

Are they implying all of the name (last, first, middle) should be caps are just the lastname?

Question 7. Other names used (including Maiden name)

(answering Maiden name) Should this be in the same format of question 1 with LASTNAME, Middlename, Firstname?

Question 6 & 8: status and prior spouses

If I put "single" for question 6, do I put "None" for 8, or can I leave it blank?

Question 12: If your fiancé is in the US...

She's not in the US right now. Should I leave that blank, or say "Not currently in the US" or something to that effect?

Question 15: Fiancées address abroad

She is in england. I'm concerned about the State/Province name. What is the proper name to put in there?

I am also assuming country is United Kingdom, not England. Correct?

Question 16: Name and address in non-roman characters.

I can put her name in original polish characters, but the address is still in United Kingdom so it would just be a 1-to-1 copy of question 15 (except for the name).

Am I correct in that assumption?

Question 18: Has your finance met with you in the last 2 years

After the yes/no check box it asks to describe how you met. We met (and became romantically involved) over the internet long before we met in person. Should I be describing how we me in person, or how we met in the first place? Or should I try to fit both in?

Check out the example forms if you haven't already (click Example Forms in the light blue bar at the top of the page). I filled out the forms by hand, and had no problems or questions. The examples show all caps for last names, but not for the rest, for example.

Good Luck!

Question 1:

Is your last name (known as family name) in capital Letters, that is how I read it from your post at least

Question 7:

Means any other names you have used other then your current one, which I would do first, middle last

Question 6:

If applying for a K1 you should probably put status as Engaged, or something similar as you are not single

Question 8:

if you have never been married before should put none in there

Question 12:

it says if....since she isn't I would assume you leave it blank

Question 15:

the state/province in the UK is the County, and The country should be put as England as the United Kingdom isn't a country it is a collection of Countries that's my opinion

Question 16:

I would Leave it blank as the address is already in English, so there is no point in translating it anyway

Question 18:

Well it is asking specifically about the meeting in person so i would stick to just how you met in person, such as When and where i,e for me it was like Mar, 2010 - I flew to Chicago
