I am petioning for my wife that resides in Italy. Our case is already in the NVC and we paid the AOS and IV Bill fees. Is there still a fee that we have to pay for the interview at the embassy in Naples?

No, after you pay the fees at NVC, there is nothing else to pay except for the medical. There shouldn't be any fees for a IR-1/CR-1 visa at the embassy/consulate.
