I presently work at a Shell gas station, and the name is just numbers and my province. A bookstore job I had, on my T4 the full name and address are completely different. Would I put the full name and address I see on my T4 or for the exact building I worked at?

I would assume you use the physical address of where you work/ed. T4/tax forms generally have different name/addresses - in my case the head office of the company i worked for.
