Is now a US Citizen immigration completed Jan 12, 2012.

I think you're referring to something that's listed on the blue slips frequently at the consulate in HCMC. It's a document listing every place you and/or your fiancee has lived for the past ten years. They would usually be looking for an address that matches the address of someone else, such as an ex-spouse or a member of your fiancee's family. This isn't an official document of any kind. It's something you would be asked to produce yourself.

12/15/2009 - K1 Visa Interview - APPROVED!

12/29/2009 - Married in Oakland, CA!

08/18/2010 - AOS Interview - APPROVED!

05/01/2013 - Removal of Conditions - APPROVED!

it is on the blue slip.

It ask that the petitioner in a signed sworn, notorized statement list your current residental address and telephone number (home, work ,and cell)include all residences for the past 10years. begining and end date of the time period at each address, and the full name and date of birth of each person residing with you at each address, along with a description of your relationship with the person (cousin,spouse,ect.....)

I plan to format it the same as a resume.
