AOS (k1) - vaccination supplement (I-693A)

Hi,

I am just looking at the documents Im going to need when I apply for AOS and it says that I will need I-693, Medical Examination. Im here on K1 visa and Im getting married in September. In the instructions it says, that I can use the vaccination supplement (I693A) from the medical exam I took abroad before the interview at the embassy. The thing is that I dont have a copy of it as they put it in the brown envelope which I wasnt supposed to open and had to give it to the officer at POE. There they told me, that they will send the documents back to the service center. So what do I have to do now? Do I still have to take another medical exam or can they review the results from the medical exam and vaccination supplement from the documents which they already have???? Please help...

We did not get a copy of our medical exam either but i have read on vj of people getting faxed copies of it. All we just did is take the vaccination supplement (I-693A http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf ) to the nearby civil surgeon ( http://www.uscis.gov/graphics/exec/cs/Index.asp?EmptyForm ) to complete the requirements (a varicella and an MMR booster is what we needed). Your actual medical exam is good for 1 year. If it is after that, you will need to get another one.

The form you were supposed to receive at the medical exam in your home country is NOT the vaccination supplement (I-693a). It is the vaccination worksheet (DS 3025). Usually you would take the DS 3025 to a USCIS approved civil surgeon here in the US and have him transfer the information from the DS 3025 onto the I-693a. Then you would mail the I-693a (in a sealed envelope) as part of your AOS package.

Since you did not get that vaccination worksheet, however, you have the following options:

1. Try to contact the doctor's office who did your K-1 medical and ask whether it's possible for them to fax you a copy of the DS 3025 vaccination worksheet.

2. If you have other proof of having received the neccessary vaccinations, take that proof to a USCIS approved doctor and have them fill out the I-693a.

3. If you don't have any other proof, go to any doctor and have them do a "titer test" (test for antibodies). If the results of that test prove that you are sufficiently vaccinated, take the test results to a USCIS doctor and have him fill out the I-693a.

4. Have all the vaccinations again (yikes...wouldn't recommend that one :no: )

You do NOT need another complete medical exam, but you DO need to send in some kind of proof that your vaccinations are up-to-date with your AOS package (preferably the I-693a, but some people here have sent in the DS 3025 and have been approved...others have been RFE'd for it, though).

The choice is up to you :)

Wow, it feels like just yesterday we were in your shoes.

I find it unbelievable that immigration hasn't resolved this confusion yet, with the question of vacc. supp. or no vacc. supp.

My wife had her medical done in Brazil with the embassy doctor and we weren't given any copies (and my wife did ask for them, and he said you won't need them). Anyway, those sealed doc's were given to the immigration officer, which gave us sealed doc's to give to immigration at POE.

We saw the same thing about the vac. supp. when we filed I-485 and didn't know what to do. I did some research on this site and listened to a VJer, who said he wrote a letter stating that he never received any copies of his medical exam and gave the sealed docs to the immigration at POE. He didn't receive an RFE, so we decided to do the same thing.

Well, we never received an RFE, the immigration officer never asked us for the vacc. supplement, and Pamela should be receiving her GC any day now.

the White Card, ha)....immigration free for 2 years!!
