Hi,

While reading the visa procedure, i came across this form DSP-122 and i have no idea whether iam gonna be filling this form out or not ?

It has some similar information comparing to form DS-230.

Please can anyone tell me what is that form useful for ?

Is it mandatory to fill it out ? is it required in my visa type ?

DS-230 is required for your visa type CR1/IR1

DSP-122 is not for you
