In reference to questio 35 on the form, it requests dates to and from the usa incl visa type.

my question is, do they request exact date and month and year, or month and year - will that suffice.

Ive been to the US around 10 times incl a previous K1. Some of the dates I can remember as its marked in my passport, but some are a big vage re the exact date of arrival or departure. I dont want to misrepresent anything, or miss a visit out - god forbid, I want to be as honest / accurate as possible on the form.

Ah, my apologies I see it now, they want mm-yyyy

In reference to the location - is that POE or the City I stayed.

An I assume Id attach an addendum for further visits on a seperate piece of paper?

Richi, your self answer (mm-yyyy) is correct

For location, city and state would be fine.

Your assumption of attachment is correct too.

Speaking of Q25, if the applicant has never visited USA, then you put "never." But on what box? From/To field doesn't accept character. I can just shove it on under Location?

I would be inclined to put 'never' in both columns. Or are you filling it out online? Yes, then, Id do that, and if/when you print it out, put 'never' or 'none', then.

Yes. Q35. I'm going to be filling out the PDF first, then print it. I can't put never on first column. But I can on the location column.
