Hi,

I'm going in for my appointment with the civil surgeon so that they can fill out form I-693 for me. I just noticed that Dr. Seiden's office did not give me anything regarding the results of my blood test (syphilis, etc). Does anyone know if that means the civil surgeon will have to do it again?

I find this part of the process so frustrating. I never got a DS-3025 either. Boo.

No, you never get the results of the blood work - that is all contained in the sealed envelope that you turned in at your interview. All you need is the vaccination record from the I-693 and the signed page - nothing else. USCIS already has the rest. It was contained in with the rest of the documents from your immigration file in the big yellow envelope you turned over at the border when you entered the US. Don't let any Civil Surgeon try to tell you that you need to redo any tests or that you need a full medical - you don't. You can print out the information that states that K-1 visa holders who had their medical less than a year ago do not need the full medical, only the vaccination page. The reason for getting the I-693 is because K-1 holders don't require all of their vaccinations prior to entering the US, but AOS applicants do. You have probably already had all of the necessary vaccinations and USCIS will have that record from your medical. You are now just providing final verification that you have satisfied the vaccination requirements for the AOS.

If it helps to reassure you, if your results had been positive, you would not now be in the US. You would be undergoing treatment still in Canada until the test results came back as negative before your visa would be issued so you don't need to worry about that.

If you have the documents you brought with you to your medical that showed your vaccination history, you can use those to show your vaccination history for the Civil Surgeon. All he is really doing is transferring the information from your records (or the worksheet if you had received it) onto the official form. It doesn't have to be from the worksheet - it can be from whatever other sources you have (Health Clinic, Family Doctor, School records, etc.) - just as long as he can see you have had the shots and when.

Good luck.

thanks Kathryn! I'm going to make a few more calls and see if I can get a lower quote than $75 for the vaccination section of the form.

$75 isn't too bad but you might be able to do better. 6 years ago I paid $50.
