I was ready to send my package tomorrow but i got really confused.There are 2 different addresses for texas lockbox.

which one is correct?

USCIS Dallas or Lewisville.

I want to send through united states post office /express ?certified ? which address should I send?

another thing.I did not include a copy of my driver's license.should I add that as well?

I don't think there is a mention of that in the package. You need to bring your DL with you on your interview day, but not a copy in the package.
