At our AOS interview on 4/6 we forgot to bring our I-693 vaccination supplement. We subsequently got a RFE letter on 4/12 asking us to bring it to the office on 4/27 @8:00 am. We downloaded a list of civil surgeons from the USCIS website and went through 10 doctors on 4/16. Most doctors would only do the full exam and not just the vaccination part. We finally found Dr. Trinh, information below, to do our supplement. He transcribed my wife's vaccination history and we paid for 4 more shots costing $255. We paid in cash because he did not accept insurance or credit cards. However, he refused to fill out Part. 5 (Civil Surgeon's Certification) after he completed the work.

Dr. Tuyen Trinh

118 Baxter Street, Suite 502, New York, NY 10013

Dr. Trinh said that he would only sign Part 5 if he completed the whole exam. He wrote in the remark's section of Part 2. that he only checked my wife's vaccine history and applied the needed vaccinations. They would not even include our Part 1. (Information About You) in the sealed envelope.

What can I do now? Time is running short.

The pricing is about right - however, he is in error about not signing the form in the proper place.

I believe you can complain about civil surgeons to the USCIS for review.

Depends on what the USCIS does with the sealed envelope - they may accept it - since it shows you had the vacs needed (and that is what the RFE was for).

I do not understand how you had the interview without the 693 sent to them first, but I see you did -weird - I would think you would of gotten the RFE for that prior to the interview date.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

WARNING: Phrases in this post may sound meaner than they were intended to be. Read the Adjudicator's Field Manual from USCIS

My wife came on a K1 visa last Oct., so she didn't need another full exam. I miss read the part that said "Bring Vaccine Supplement" and thought that her vaccination booklet was good enough. The instructions didn't say it had to be Part 2 of I-693. Otherwise I'd have brought that.

The supplement can be done by any health clinic or doctor. We had ours done at our health clinic for 23 bucks ( 3 shots ). They were not sure of how to do the form but after I explained they signed & stamped it.

At our interview the officer simply said " you look healthy to me" & didnt want to see the form. ? I would just take the forms you got from the doctor & they should accept them.

The CS knew I was right about the TB test being unnecessary, but took

the opportunity to get a $120 fee and ran with it. I think the majority of

them are like this. The USCIS disavows any attempt to police them or

accept any complaints regarding this.

I really wouldn't have handed over the cash until he finished the work! If he wasn't going to sign off on the darn thing! What a %$*(%^#! :ranting:

Anyway, we got RFE'd because they didn't sign part 5, so part 5 is definitely necessary. Guess you'll have to keep looking for another civil surgeon who will do the form correctly, and stop paying people for not doing the job....

Well he stamped and signed Part 2... I called USCIS up and they said to just bring it in and bring a letter explaining the situation. But that's just from their customer service operator. It's really up to the individual handling the folder.

I don't know about just any old clinic... USCIS states it has to be a civil surgeon. I think whoever got it stamped at any old clinic got lucky they didn't check too hard. In any case, we've already passed the interview, this is just making sure our folder is complete so they can send out the card.

If anyone knows a GS in NYC that would transcribe our record for low cost, I would definitly take the time to go and do it. Or if they can do it by fax and express mail it quickly, I would also do it.

Guess you just got lucky too from what you say. The difference in the health clinic luck & yours seems to be 232 bucks.

yeah but they aren't suppose to have just anyone sign it. You got really lucky that they approved it from the health clinic as the instructions specifically state is has to be filled out by an approved civil surgeon.
