Hi,

My wife had recently completed her requirements for getting all of her shots, physical, etc. We were given a sealed envelope that has her vaccine records inside and is it to be mailed to the USCIS. Does anyone know if we just mail this to the USCIS office that had orginally requested the vaccine form be completed or does it need to be mailed to a different USCIS office. I am having trouble contacting the USCIS so thought I would ask here. Thanks for the help

Your wife have already in the country? What type of visa? Please fill up timeline in order to let other members can help you out.

USCIS address should be Chicago Lockbox as your AOS addressed.

Did you receive and RFE for the request for this information? If so it should have the information on where to send. Assuming it is in connection with AOS it would likely be the Chicago Lockbox where AOS was sent but really need more information.

The sealed envelope with the I-693 from the USCIS civil surgeon should be mailed with the your AOS application.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

Yes it should, but they have recieved an RFE for it, so this is a little late.

I would think there would be an address on your RFE as to where to send it, but I confess I'm not really sure as I haven't gotten an RFE to see how the letter looks.

Actually we had gotten a letter requesting the vaccine record must be sent in so that they could proceed with her Permident Resident I-485 form that we had submitted. What I was wondering is do we just send this to the same USCIS office the request came from, or should it be a different one. Thanks

Theres another thread http://www.visajourney.com/forums/topic/250757-where-to-send/ in which someone got a RFE for the medical and was advised to send the information in to the office the RFE came from with the orginal RFE (and to keep a copy for yourself of the RFE)
