On the 325a Spouse biographic form, it starts out with (family name) (first name) (middlename)

My wifes name is Gergina Nunez Arn***ar she is from Peru. Nunez is her fathers last name. Arn***ar was her mothers last name.

My lawyer told me to fill out the form (family name) Arn***ar (first name) Gergina (middlename) Nunez

She is getting a passport and she says her name will be Gergina Nunez Arn***ar de Bishop (her new Name after marriage)

My question is how do I fill out the form now ? (family name) (first name) (middlename)

My ex lawyer said I need to have her name as her passport is ? I am confused with the middle name since she does not have one, and now adding my last name Bishop , she adds it in Peru as de Bishop. I hope someone can help me, I just want to have this right so she does not have to send me her documents twice. Thanks so much, Jimmy

She needs to decide what her name is going to be. Two family names is fine, such as Nunez de Bishop. Fill out the form with the name that will be her current legal name at the time you file the petition.

fill out the form with the names as in the passport, if she does not have a middle name, don't put the last name as middle name. It's fine not to have any middle name, I don't have one either...

The OP indicates the passport name will be Gergina Nunez Arn***ar de Bishop. Are you suggesting her family name is three of the four? Actually the passport will clearly state just which portions of the total are the surname. If more than one name remains, then only one is the first name and any others are "middle" names.

She is getting a Peruvian passport. Her new DNI card has her name Gergina Nunez Arn***ar de Bishop I assume her passport will be the same, I will repost in a week when it comes in to be sure. I am not sure of what to put on her family name. Nunez is her dads last name, Arn***ar was her mothers last name.Bishop is my last name, but they add the de.

Before we married her names was Gergina Nunez Arn***ar

The lawyer had (famly name) Arn***ar (first name) Gergina (middle name) Nunez

The opening of the 325 form has (famly name) (first name) (middle name)

If her passport will say Gergina Nunez Arn***ar de Bishop how would you fill it out ?

She has no middle name but 3 last names possibly, this is killing me. HA HA

Thanks for being kind in the responses, was afraid of ridicule.

It depends on which parts of her name show as "surname" on her passport. It will become clear when you see the passport.
