Need help with DS-230

Hi You all,

I'm helping my Mom fill out DS-230. My Dad pass away almost 2 years ago so , I checked her as a widowed, but in question 14 are asking for his information, should I put it or not? and specify that he is deceased? or should I leave this part blank?. I'm confused :wacko: because in question 19 and 24 they're asking about my grandparent's and says "If Deceased, Give year of death.

Please any help!!!
