I am a from Philippines but processed my Fiancee visa at Hong Kong. I wasnt given any copy of my medical, instead gave me a sealed envelope for US Consulate. The only thing i have is the receipt. Planning to have my vaccination done next week. Would it be a problem if i dont have a copy of my medical with me...

Don't worry, the St. Lukes Medical Clinic will send them to the Embassy. :)

Dec 15,2009 - Assigned interview date on January 24,2010!!

Jan. 24, 2010 - Interview!! Approved

Jan. 28, 2010 - VISA received

Mar. 03,2010 - POE, New York (JFK)

Apr. 12,2010- CA ID

May 20,2010- CA Driving License

May, 25,2010-Wife told me that she is pregnant ^_^

Jan.25,2011- Our daughter was born

Did you also have your medical exam in Hongkong?

Do you remember the name of the vaccines given to you? Do you have a copy of the vaccination record?

SLMCEC didn't give us a copy of our medical exam results but handed us DS-3250 and the X-ray CD.

If you were not given a copy of your vaccination chart, but have a copy of your vaccination records, then that could work, too. If you haven't had shots of tetanus, then you might need to get that. My civil surgeon said the USCIS can be nit-picky on that.

If you were not given a copy of your vaccination chart, but have a copy of your vaccination records, then that could work, too. If you haven't had shots of tetanus, then you might need to get that. My civil surgeon said the USCIS can be nit-picky on that.

I had my husbands vaccination records but wanted to be safe, so I emailed the designated clinic where he received his medical. They asked for a signed letter from my hubby authorizing him to receive a copy of his vaccination (DS-3025). They emailed it the next day. Funny thing to me is that at the bottom of the DS-3025 it clearly says "Give copy to applicant". Seems from reading on here they almost never do. I believe its your right to have a copy of it. Try emailing the place where the medical was performed to see if they will give you a copy. When requesting it, make sure the beneficiary signs a letter saying its okay to release the record.. Hope this helps.
