Did you look online at your local Indian Consulate? They will have the application online and other requirements. You could also make a phone call on Monday to ask directly.

What you really should do is apply for an Overseas Citizenship of India (OCI) card, then you can leave to India whenever you want and stay as long as you want, no visa required. :thumbs:

Sorry, I read you post too quickly. However, I think you can show your Indian birth certificate (or other documents like 10th class, proving your birth and parentage) instead of passport. You might have to call up the Consolata or go in person with your birth certificate and other documents (surely, you have copies of your lost passport? You can use those) and ask them, maybe they will let you apply with no problems. Good luck! :thumbs:

but applications for OCI are accepted from residents of the following States and Territories: New York, New Hampshire, New Jersey, Maine, Connecticut, Massachusetts, Ohio, Pennsylvania, Puerto Rico, Rhode Island, Vermont and the Virgin Islands. and i live in CA.

(Consular Jurisdiction: Alaska, Arizona, California, Colorado, Hawaii, Idaho, Montana, Nebraska, New Mexico, Nevada, Oregon, Utah, Washington, Wyoming)

Do check the consulate service menu and they do have OCI listed there.

No, you don't need any visa, you are an Indian national. For how long you can stay, India won't stop you for however long you want to stay but since you are a Green Card holder, you might have problem entering US if you stay in India for more than 6 months.

Being a GC holder of US, you will have to satisfy the residency requirement of US govt.

You may need the I-751 extension letter (if you have received it by June, take it).

(edit: I looked at your timeline, and you aren't even close yet to I-751; the conditional-GC will do)

I-751 is for removing condition, which you will have to file at 90 days mark before your 2 year GC expires. So you don't need it now, you have time for it, relax and enjoy your holidays for 2 weeks in India.
