Good morning vjers!!! I just called the nearest civil surgeon recommended by USCIS site in our place and the price is so expensive, way expensive than my other friends at other states... Here they charge $350 plus if you need another shots and there is additional fee per shot.... Its far expensive to all the fees that I have read in here at vj...

Can someone tell me where did you had your transcription in Washington? that's of course cheap... My friend had hers for only $30 but she's in Tennessee... I would appreciate your inputs... Thank you in advance... God bless

that's really expensive...double check if that's just for transcription and not a new medical exam package, that's the average price for med exam here in RI

Yes, definitely call around some more.. even the ones that might just seem a little far off. I'm in the Tacoma area and I call some that were close and only to find out when I got into the office, that they were trying to charge a different price or acted like they never said what they said to me on the phone ($50 - $80 on phone, then when I got there, $150 - $175)... talk about wasting my time.

I got into an argument with one lady that said that the DS-3025 form was not sufficient enough as proof of what shots she got and that my wife had to show actual shot records... lol... I almost wanted to just drive up there and call her an idiot... before hanging up I just told her to go to the USCIS site and print up the procedures on how to handle the I-693 as she seems to be out of date on it(was said in a nastier way as I was really getting irritated with her)

If you are looking for a cheaper one, there is a CS lady out in Lacey, WA... I think she is the only one listed there, that will transcribe for just $50, I think... and another one out in Federal Way that will just transcribe for I think $70... You have to make sure all your shots are up to date before having them do it

I ended up using Dr. Arnold (before she retired, from what I heard), and this was after contacting those two CS I just mentioned as I was really getting fed up with the varying in prices for transcription...

We already sent in our AOS/EAD/EP and got our NOA 1 already, so hoping that we don't get a RFE for the I-693 for going out of state.

Here are the two CS I mentioned.. if we get an RFE for the I-693, we'll end up going to one of them.

We are located in Everett. My husband had his medical done in Lynnwood. We called everywhere and finally found this place. It cost $155. I am sorry but dont remember the doctors name. If you take the I=5 and get off on the 196th street exit and go west. Just pass evergreen, hwy 99, there is a chinese restaurant on the left and just pass that is a drs office. Thats it. It is across the street almost from Trader Joes. Hope that helps.

It would be worth the drive to check it out. We had it done in June so price is still probably the same.

Hi teepee,

Call the civil surgeon on Rainier Avenue South. Her name is on the list of the accredited CS on the USCIS website. I think she only charged us for less than 50 USD for transcription. I didn't have any additional vaccinations.
