Ok. We have a copy of the Vaccination Documentation Worksheet. Do we need to submit I-693? He had his medical exam and blood test and TB test done for the K1. And from what I understand we do not need the medical exam again. However the Civil Surgeons I have spoken with all say that we do. Should I just submit a copy of the VDW instead of the I-693?

Many have been successful submitting only the DS-3025... however, some recent reports of RFE's for some who have done this. Theoretically if the DS-3025 was included in the sealed POE packet an adjustee is suppose to be OK... recent RFE's may indicate the USCIS is not adjudicating this exactly this way

I think this one would be good for a INFOPASS to the local immigration office to ask theses question to?

That is if it has to deal with a U.S. passport and/or Greencard.

Go to USCIS.GOV and follow the path for INFOPASS and set up a appointment at your local office for her.

NOTE: Take all the Documents you can to support the Robbery and the old passport .. the document to show the error on the OLD one etc.

You never stated if she is from the U.S. or what country she and you are from so I don't know the Closest one to you.

My wife is from the Philippines and every time we have deals with her passport of something to do with the Philipppies we have to fly to Chicago IL.

Laughing .. I know it is hard to think of all the details so that is why we asked the quesiton to help you along the right way.
