I am just beginning the AOS phase and VERY VERY confused by the whole medical portion of it. My wife received Tdap, MMR and Varicella at her medical in November and is shown on her DS-3025. She is 32 years old and checking the newest required vacination list (updated Dec. 14 2009) she is only required to have those 3. So...do I not have to file the I-693? That would be terrific if I dont because I really dont want to have to deal with finding a civil surgeon who wil only charge to fill out the form.

if her shots are up to date then she only needs to send a copy of the vaccination she had for her k1 medical in her home country.

BUT its up to the person who gets your case if they want to request you to send an I693 so they may ask for one they may not. Many people have not been asked for one but also many have been asked for one. No one can say for sure if you will get an RFE or not, you can go ahead and try sending it and see what happens. Or you can go ahead and get the 693 for the " just incase " factor.

Or send it in without I-693, but research one in your area (that doesn't overcharge as much) in case you get an RFE. That's what we're doing.

Personally, I don't think it is worth the risk. Many people have been approved without the I-693 in the past, but there has been a recent wave of RFE's issued to individuals who did not enclose the form. If you just look around the AOS forum here, you will see that a great number of threads have been posted on this topic, all with individuals who have received RFE's for the missing transcription. There is a possibility you will be approved without it, however there may be an even better possibility that you will not. Also consider that if you end up with an interview, the I-693 is on the list of required documents to bring to the interview. It seems easier to have this done now, then scramble later in the process to find a willing CS. Just my personal opinion. I waited until I had my interview notice to get the I-693 done and it caused me a lot of stress and aggravation due to the time constraint.
