Hi everyone

I'm a k-1 visa holder, I've applied for AOS I send all the documents that I must send, but they recently ask me for an I-693, I read the form but I don't understand so many things, I wonder if I can't fill it by myself or a sugeon must fill it for me?

When I went to the appointment the Embassy asked me for all that stuff, I don't know exactly what test that I did but I know that I did all the requirements.

The last time I've tested for that purpose is July 28, 2009.

must be signed by a USCIS civil surgeon, so you might as well let the surgeon prepare it

Do you have a copy of the test result you had in Canada? If you have, you need to bring it to a surgeon so they will know what vaccines you still need but if you don't have, you still need to go. They will do a blood work and by that they can determine what vaccines you still need. The result will be in I-693 form signed by the surgeon and will be given to you in a sealed envelope.

I still have an envelope with a CD outside I didn't open it and I have a copy of the vaccines that I took... Do I still need a surgeon? and the Instuctions tell that I don't need an another examination if the one that I took has less than one year....

Many people have successfully adjusted simply by including a copy of the DS-3025 to the I-485 application... It is what we did.

The cd is for the chest x-ray. The copy of the vaccines is what you need. I understand that you submitted already your AOS but USCIS asked for the I-693 right? Like what payxibka have said, some people only submitted the DS-3025 (which is the vaccine result that you have now) instead of I-693. It is your choice. You can send that or go to a surgeon have that result be transcribed to form I-693 and get the remaining vaccines that you need ( which will be included in the I-693 too).
