Hi,

I've been reading about the medical requirement for AOS to submit and m Confused!! I have a copy of my DS3025 and I plan to submit that in my AOS package... however Ive been reading here and some people is saying that that I have to submit the I693 form???

Reading Captain Ewoks post it says:

"If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693."

I just had my medical last August 2009 so is it okay not to submit I-693 and just my DS3025???

THANKS!!

Some people do and some people don't, its possible to still get an RFE for the I-693. I have sent my AOS in oct 1st without the I-693 because I couldn't obtain one at the time. After I was able to get one I have it at home, incase of RFE for it, only cost 37$. I-693 is a vaccination transcription by a civil surgeon in the US, stating that all your vaccinations are completed for the AOS process.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

Getting your vaccination records transferred by a Civil Surgeon isn't always an easy process. Seems to depend on geography, but some CS insist on carrying out the entire new medical, at the full medical cost of several hundred dollars. There is a CS who will transcribe for a reasonable fee by mail. You simply send off your vaccine records and he returns the I693 completed, signed and sealed.

Captain Ewok's post states that you can send your DS3025 in place of the I693, which is another way of saying instead of.

I'll have a search of the threads for you to see if I can find the CS' details. I know they were discussing this on the UK forum recently. Back in a sec! lol..

Take note that on your DS3025 is probably shows that some of the required vaccinations were not available (TD and varicella). If so, your vaccinations were not completed. You will need to get the missing vaccinations and show the vaccinations are completed on the I-693.

What if the beneficiary has already had chicken pox (varicella being the vaccine for chicken pox).

Then the panel physician can annotate that the vaccination is not required rather than not available.
