I sent my vaccination information off to Dr. Arnold on Friday. After calling several local civil surgeons, I couldn't find one willing to fill out the I-693 without administering a new medical. I would suggest calling some local doctors first, that way if they question why you didn't use an in state doctor you can tell them you tried.

I sent off my AOS application without the I-693, but I have been asked to appear for an interview. As you probably know now, if you do not send the I-693 and do not receive an RFE but end up with an interview, you will need to bring the form to the interview with you. It is listed on the interview letter as a required document. So for those who are going to file AOS soon, I suggest sending the I-693 in with the application.
