requirements for transcription of vaccination records

Hello everyone!

I have my vaccination supplement form from St. Lukes and got my 2nd doses of shots here in the US already. I am wondering about the medical x-ray cd, did you have to submit that together with your immunization records to the civil surgeon for the transcription to the I-693?

I really would like to hear your input about this. Thank you! God bless us all!

I mailed AOS package for my wife and we only had to take the immunization record to the civil surgeon. He filled out I-693, signed and sealed the envelope.

I asked this question before but noone answered. one more try-do i need to submit recent results of PPD test (tuberculosis skin test) to a Civil Surgeon?
