Hello fellow vjers

Iam preparing my ds-230 form, i had a question regarding line 2 were it states other name /alias, in my passport and birth certificate i have the same last name, in my Canadian driver's license and school id , health card and my social# i have the same last name but one of the letter in my last name is different.

Example: passport and birth certificate last name: Chowdhury

: drivers license and other id: choudhury

do i put this last name on my drivers license on Qestion#2

just use the one on passport and birth certificate

there are some cases when you write foreign name in English there are more than one way to

write in English. they understand this, i don't think they will many any deal out of this

Agreed, put what is in your birth certificate.

If there is not a particular reason why the name are different on your documents, I suggest you get them fixed so all relevant documents have the same name, it will help you in the future and prevent confusion

There is no reason the name's are different on my document when i started going to grade school the school made that error and that got transfered to my other schools, and uni, and i was just to lazy to fix the error on my name, by the way thank u all for the reply. as long my passport match's my birth certificate and my marriage certificate im good. This what the layer told me.

You have to write the other name in the space provided below because you've been using this name as well.

All names, including different spelling must be included and appears on your PCC as well.

Dec 15,2009 - Assigned interview date on January 24,2010!!

Jan. 24, 2010 - Interview!! Approved

Jan. 28, 2010 - VISA received

Mar. 03,2010 - POE, New York (JFK)

Apr. 12,2010- CA ID

May 20,2010- CA Driving License

May, 25,2010-Wife told me that she is pregnant ^_^

Jan.25,2011- Our daughter was born
