I am pretty sure that this form needs to be filled out in English.

It would be in English unless there is a specific requirement by the Embassy. For example for Russia, the DS-156K form is all in Russian and must be filled out in Russian. I am not sure about Ukraine.

This is what I got from the U.S. embassy website for Ukraine.

Your fiancé(e) will fill out a Nonimmigrant Visa Application DS-156 in duplicate, as well as one copy of DS-156K form. Each dependent child also needs Nonimmigrant Visa Applications DS-156 in duplicate. Original documents plus one copy together with a notarized/certified English translation thereof, should be brought to the interview. Originals of primary documents, such as birth, marriage, and death records, will be returned to the applicant after the interview.

Maybe someone that has been to the Ukrainian Embassy will answer soon, so that you can be sure.

Good luck.

The forms the consulate sends to beneficiaries are in Ukrainian and Ukrainina is acceptable for the DS-156 and DS-156k forms. Any documents must be translated to English (birth certificate, police certificate, etc.) We never got the packet 3 forms, but I have seen them from others. We used downloaded English forms. I completed all the forms and Alla only signed them. Alla never touched a form in this entire process except to sign them, not because she can't speak English, she does very well (corrects ME all the time).

When doing the interview or turning in documents they will ask if she wants to speak English, Russian or Ukrainian

Baron, the forms can be filled out in English also. You can download the forms. What they send a Russian beneficiary will be in Russian. The Ukraine consulate sends out the forms in Ukrainian which many Ukrainians do not speak. But it is the "official" language. I filled out Alla's and the boys in English

you can fill in English, in Russian or in Ukrainian, but it could be much better to fill in English.

About my interview, no one asked me what language I prefer to speak with CO. He started talking only in English.

At Alla's that was the first thing they asked...which language she preferred. Oh well.
