hullo again folks..please help me in this question.with the adjustment of status application,can i just submit the vaccination document i got from st lukes?

Yes, you can submit a copy of it but you need your vaccination records transcribe in I-693.

i was also thinking though if it necessary to transcribe it since my friend who got approved her greencard didnt need to transcribe it,she just sent her supplemental vaccination record from st. Lukes.

some people just submitted their ds vaccination supplement and they got approve without any rfe for the I-693, but some people got the rfe, so it's up to you.

from the guides (one of them tabs at the top of the page):

I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.)

the form you should have received from your medical exam in the philippines is a ds-3025, which is transcribed by a civil surgeon in the usa. you will get a sealed envelope to submit with your aos package. as with the packet you received from the embassy/consulate, do not open it!
